Customer Proprietary Network Information
(CPNI)

Protecting the privacy of your service and usage records is your right and our duty under federal law. Mid Century Telephone Cooperative, Inc. is required to inform you that you, the customer, may direct us not to use any of your customer proprietary network information (CPNI) to offer you additional categories of products and services.

What is CPNI? CPNI includes personal information concerning the phone numbers you call, the length of phone calls, and services you purchase. If you do not restrict our use of your customer information, we may use your customer information to offer you products and services that you may find to be valuable additions to your existing services. Mid Century will only share information about you within our family of companies so we can serve you more efficiently. Those Mid Century companies are:

• Century Enterprises, Inc. (Long Distance)
• Cen-Satt, Inc. (High-Speed Internet)

Mid Century will also disclose CPNI upon your written request to anyone you designate.

Restricting your information will not affect the products you currently have through Mid Century and may not eliminate other marketing contacts that are allowed by law. Even if you restrict the use of your information, Mid Century may use it to discuss additional available services with you when you call us. Your election is valid until you affirmatively revoke or limit it.

If you do not want Mid Century to use your CPNI to offer you categories of products and services to which you do not currently subscribe, please complete the CPNI Opt-Out Form or, you may call our special toll free number    1-866-212-6698 or locally 778-8777 to restrict or un-restrict CPNI. These numbers are available 24/7 for your convenience.

Under no circumstances will Mid Century share CPNI information with another party, regardless of whether you restrict use or not.
