Privacy Statement
Mid Century Telephone Cooperative, Inc. (MCTC) recognizes that the trust of our subscriber’s security and electronic privacy requires responsible privacy protections. We respect and protect the privacy of our subscribers. As a provider of telecommunications and related services and products we recognize that we must maintain the confidentiality of every subscriber’s telephone calling and other account information. We also protect the privacy of our web visitors. Additionally, we have an obligation to assist law enforcement and other government agencies responsible for protecting the public welfare, whether it be an individual or the security interests of the entire nation. If and when we are asked to help, we do so strictly within the law and under the most stringent conditions. Mid Century’s Privacy Statement describes how we will protect the personal privacy of our subscribers.
Personal identifying information we collect and use
Mid Century may collect personal identifying information regarding our subscribers, including information customers give us, information collected as a result of the subscriber’s relationship with us, and information we obtain from other sources. Examples include name; address; e-mail address; telephone number; billing, payment, usage, credit and transaction information (including credit card numbers, account numbers and/or social security number); and demographic information.
We also collect personal identifying information that our web visitors choose to provide to us (e.g., name, address, telephone number, e-mail address) when registering on our web site; ordering MCTC-offered products or services; sending us e-mail; responding to our surveys; entering contests or meeting registrations; or in connection with online ordering or billing functions.
We use the personal identifying information of a subscriber to provide, confirm, change, bill, monitor and resolve problems with the quality of MCTC-offered products and services. We may use the personal identifying information of a subscriber or web visitor to develop, market and sell our products and services.
We may aggregate the personal identifying information of different subscribers or web visitors to produce data about a group or category of services, subscribers or web visitors. For example, we might use aggregate data about the types of services our subscribers have generally purchased at the same time in order to develop attractive bundled service offerings. Such aggregate data, however, will not reflect any personal identifying information of any specific subscriber or web visitor.
Who sees the information collected by Mid Century?
Mid Century considers the personally identifiable information contained in our business records to be confidential. We may disclose personally identifiable information about you to others (such as our employees, contractors, and agents as well as outside auditors, professional advisors, service providers, and regulators), either with your written consent or without your written consent as authorized by law, if necessary to render our service or to conduct a legitimate business activity related to a service provided by us to you. We may also disclose to an affiliate company, Century Enterprises, Inc. (Mid Century’s long distance affiliate) or Cen-Satt, Inc. (Mid Century’s satellite high speed internet affiliate) your name, address, or other subscriber information. Mid Century and its affiliate companies may use the information collected in order to:
· Assist us in providing administrative and other services.
· Prepare, print and/or deliver monthly invoices for our services or other marketing or informational materials to our customers.
· Prepare and conduct subscriber surveys to assess and enhance the services that we provide to our customers.
· Collect outstanding fees and charges.
· Market our products and services (subject to your right to limit or restrict us from making these offers as described in the CPNI Policy or other applicable law).
· Assist us in detecting and protecting against fraudulent, abusive, or unlawful use of, or subscription to, our services.
The frequency of any information disclosure varies in accordance with our business activities and needs.
Personal identifying information we disclose to third parties
Mid Century does not provide personal identifying information (other than information included in our directories and directory assistance service) to third parties for the marketing of their products and services without your consent.
We may, where permitted or required by law, provide personal identifying information to third parties (including credit bureaus or collection agencies) without your consent:
· To obtain payment for MCTC-offered products and services, enforce or apply our subscriber agreements, and/or protect our rights or property.
· To comply with court orders, subpoenas, or other legal or regulatory requirements.
· To prevent unlawful use of communications or other services, to assist in repairing network outages, and when a call is made to 911 from a subscriber phone or information regarding the caller’s location is transmitted to a public safety agency.
· To notify a responsible governmental entity if we reasonably believe that an emergency involving immediate danger of death or serious physical injury to any person requires or justifies disclosure without delay.
A subscriber’s name and telephone number may also be transmitted and displayed on a Caller ID device unless the subscriber has elected to block such information.
Can I prohibit or limit Mid Century’s use of my personally identifiable information?
If you do not want to receive telemarketing calls from Mid Century or its affiliates, you may call 1-877-643-2368 or 309-778-8611 and request to be placed on Mid Century’s Do Not Call list. Please note that a Do Not Call request will not eliminate all telephone calls to you from Mid Century or its affiliates as we may still continue to make non-telemarketing account-related calls to you. We comply with all applicable laws and regulations regarding Do Not Call lists.
Customer Proprietary Network Information (CPNI)
In the normal course of providing telecommunications services to our subscribers, the information that we have (1) relating to the quantity, technical configuration, type, destination, location, and amount of your use of telephone service, and / or (2) contained on your telephone bill concerning the telephone services that you receive is subject to additional privacy protections. That information, when matched to your name, address, and telephone number is known as “Customer Proprietary Network Information,” or CPNI. Examples of CPNI include information typically available from details on subscriber’s monthly telephone bill -- the type of line, technical characteristics, class of service, current telephone charges, long distance and local service billing records, directory assistance charges, usage data, and calling patterns. Your telephone number, name and address are not CPNI because that information is protected by the general privacy policy described above.
Protecting the confidentiality of your CPNI is your right and our duty under federal law. We do not sell, trade or share your CPNI – including your calling records – except as may be required by law or authorized by you.
As a general rule, we are permitted to use CPNI in our provision of telecommunications services you purchase, including billing and collections for those services. We are permitted to use or disclose CPNI to offer telecommunications services of the same type that you already purchase from us. We may also use or disclose your CPNI for legal or regulatory reasons such as a court order, to investigate fraud or to protect against the unlawful use of our telecommunications network and services and to protect other users.
Mid Century and its affiliate companies, Century Enterprises, Inc. (Mid Century’s long distance service) and Cen-Satt, Inc. (Mid Century’s satellite high speed internet service), offers many communications-related products and services. From time to time we would like to use the CPNI information we have on file to provide you with information about our communications-related products and services or special promotions. Our use of CPNI may also enhance our ability to offer products and services tailored to your specific needs. We would like your approval so that we and our affiliates may use this CPNI to let you know about communications-related services other than those to which you currently subscribe that we believe may be of interest to you. You do have the right to restrict this use of CPNI. Written notification is mailed to our subscribers prior to certain marketing campaigns and also every two years in accordance with federal law. The written notification allows an opportunity for you to “Opt-out”, which refers to a method of obtaining your consent to use, disclose, or permit access to your CPNI. If we do not hear from you within 30 days of this notification, we will assume that you approve our use of your CPNI for the purposes of providing you with information about other communications-related services. You may disregard this notice if you previously contacted us and denied use of your CPNI for the purposes described above. You have the right to disapprove our use of your CPNI, and may deny or withdraw our right to use your CPNI at any time by calling our CPNI voice mail line at 1-866-212-6698 or 309-778-8777, or our business office at 1-877-643-2368 or 309-778-8611. You may access our Opt-out form found on our website www.midcentury.com.
Mid Century will honor any CPNI restrictions for our affiliate companies, Century Enterprises, Inc. and Cen-Satt, Inc.
Mid Century also offers various other services that are not related to the communications services to which you subscribe. Under CPNI rules, some of those services, such as Mid Century’s video and high speed internet (DSL) services, are considered to be non-communications related services.
Occasionally, you may be asked during a telephone call with one of our representatives for your oral consent to Mid Century’s use of your CPNI for the purposes of providing you with an offer for products or services not related to the telephone services to which you subscribe. If you provide your oral consent for Mid Century to do so, Mid Century may use your CPNI for the duration of such telephone call in order to offer you additional services.
Any action that you take to deny or restrict approval to use your CPNI will not affect our provision to you, now or in the future, of any service to which you subscribe. Any denial or approval for use of your CPNI outside of the service to which you already subscribe is valid until such time as your telephone services are discontinued or you affirmatively revoke or limit such approval or denial.
The CPNI Policy above applies to our voice services.
Web usage information and e-mail practices
Mid Century may use web usage information to facilitate and enable the functioning of our web site and to expand and improve our web visitors’ online experience. When you pay your bill online via MCTC’s Estatement, our registration forms will collect personal contact and financial information from you. Our web site only collects personal information that you provide directly about yourself. Mid Century may contact you when you deem it to be necessary in connection to the service offered herein. In addition, upon your request, we may e-mail service-related communications to you but you may decide to stop receiving these communications at any time.
When you pay your bill, we record detailed information of the payment history and the methods utilized to help identify fraudulent credit card use. We may share this information with the financial institution that issued the credit card with which the bill was paid, or with law enforcement authorities, if they determine use of the credit card was fraudulent. We do not store personal information in cookies. Your personal information is protected with a User ID and Password that you and MCTC manage. You can cancel enrollment or change your enrollment information in our online service at any time. Upon cancellation, your account will become inactive.
We do not provide your e-mail address to third parties for the marketing of third-party products without your consent.
How we protect your information
Mid Century Telephone Cooperative, Inc. restricts access to nonpublic personal information about you to those employees who need to know that information to provide products or services to you. We maintain physical, electronic, and procedural safeguards that comply with federal standards to guard your nonpublic personal information.
